Dr. Cranney Family Medicine is a small membership-based practice serving children, adults, and seniors in American Fork, Utah County, and surrounding communities.
Membership provides convenient access to Dr. Cranney through office visits, telemedicine visits, telephone calls, and secure messaging—without visit copays or unexpected bills from our office.

How Do I Become a Member?
Select Schedule an Initial DPC Appointment and choose a convenient date and time.
During this appointment, Dr. Cranney will explain how the practice works and answer your questions. If you decide to join, he will review your medical history, obtain baseline vital signs, and request prior medical records when appropriate.
There is no charge for the initial appointment if you decide that membership is not the right fit for you.
Founding Membership Rates

Adults ages 19–59: $95 per month

Adults age 60 and older: $125 per month

Children ages 0–18: $40 per month*
*Children must have at least one enrolled parent.
*A one-time enrollment fee equal to the first month’s membership is due at sign-up.
*Founding members keep their founding membership rate for as long as their membership remains continuously active.
What is Direct Primary Care?
Direct Primary Care is a membership-based approach to healthcare that allows patients to work directly with their physician without the barriers of traditional insurance-based primary care. Members receive convenient access to care through office visits, video visits, telephone visits, and direct communication with their physician.
Dr. Cranney intentionally limits the practice to approximately 250 patients in order to provide greater access, longer appointments, and more personalized care.
Do you “take insurance?”
We do not bill insurance for care provided through the Direct Primary Care membership. Your monthly membership covers included services from our office, so there are no visit copays or unexpected bills from us.
Membership is not health insurance. We encourage patients to maintain insurance or another plan for major medical expenses, hospital care, specialists, emergency services, medications, imaging, and other care outside our office.
When outside laboratory testing or imaging is needed, you may use your insurance when accepted by the outside provider or choose discounted self-pay pricing when available.
What if I have Medicare?
Dr. Cranney has opted out of the Medicare billing program. As a result, patients who are enrolled in Medicare are welcome to join the practice and receive care through the Direct Primary Care membership model.
Traditional Medicare, Medicare Supplement (Medigap) plans, and Medicare Advantage PPO plans generally work well with our practice.
Patients enrolled in Medicare Advantage HMO plans should be aware that Dr. Cranney is not part of those provider networks and may not be recognized as an ordering or referring physician for certain services outside the clinic. If you have a Medicare Advantage HMO plan, we recommend contacting your insurance carrier to better understand your specific coverage and network requirements.

What if I need a specialist or a hospital?
In providing high quality medical care that is quick and convenient, many times we are able to prevent or reduce the need for hospitalizations and specialty referrals. If the need does arise, we will coordinate closely with any outside specialists and hospitals, providing you with the best possible care. We are not affiliated with any large healthcare systems, insurance companies, or large medical corporations. Thus, we decide together who will best serve your needs. We do encourage you to carry insurance or have a plan for unexpected large medical costs.

Are there any additional costs?
Possibly. At Dr. Cranney Family Medicine, we provide everything we do for the membership fee. However, for any outside services such as labs, pathology specimens, prescription medications, and imaging - these do require additional cost. Fortunately, we have pre-negotiated steep discounts with local vendors and have completely transparent pricing. Dr. Cranney can also draw many laboratory specimens on-site.

Frequently Asked Questions
What is the co-pay?
There is no co-pay and there never will be. Co-pays are created by insurance companies as a disincentive for seeking medical care. Instead, we provide everything we do for a single membership fee. We don’t believe in creating barriers for members seeking the care they need. After all, that is what we are here for; convenient and accessible care when you need it.
What if I need to cancel my membership?
Hopefully this never needs to happen, but if it does, we have made it easy to cancel at any time. All we ask is that you submit your cancellation 30 days in advance. You will no longer be billed and your membership will expire. Should you choose to rejoin Dr. Cranney Family Medicine, we reserve the right to charge a re-enrollment fee.
Are prices guaranteed?
Founding members retain their founding membership rate for as long as their membership remains continuously active.
Rates for future members may change. Any applicable membership rate and enrollment fee will be clearly disclosed before enrollment.
Why would I purchase a membership plan when I have insurance?
There can be unique reasons to enroll with Dr. Cranney Family Medicine for each person.

By purchasing health CARE directly through a membership model, it is typically much less. Prices for all services are known in advance. Just think, one unexpected trip to the ER would likely cost more than an entire year of membership.
